After submitting the form be sure to call your primary physician and previous pain management providers, if any, to have them send all of your medical records as well as a referral to Pain Management. They can fax us this information at 716-671-9079. Once ALL documents have been received, we then review your case and make a determination if you would be a good fit with Paradigm Family Health. New patient openings are dealt with on a first come first serve basis for those who have fulfilled all of those requirements.
