LOOKING TO BECOME A NEW PATIENT?
Thank you for your interest in becoming a patient with Paradigm Family Health of WNY. You can confide in one of the top pain management practices in New York to mitigate your acute and chronic pain. It is our goal to do whatever is best for our patients. We do have a vetting process to becoming a new patient. During this process we get to know you, the pain you're experiencing, as well as a comprehensive list of your past medical records. Please note at this time, due to New York State regulations, we are unable to see ANY patient with an active workers compensation or no-fault case. Please click the button below to get started.
WHAT NOW?
After you have filled out our new patient packet we will need to also receive a few things from your current providers. Most offices will fax these requested documents directly to us per your verbal request. All documents can be forwarded to us at 716-608-3788. Please reference the list below for what we are requesting in case you didn't write it down when you filled out the new patient packet.
Primary Care Provider:
- Referral to pain management
- All medical records
- Any imaging on file
Past Pain Management Provider:
- All medical records
- Any imaging on file
If you see any specialists not listed above, please have them also include their medical records as well.
ALREADY SUBMITTED EVERYTHING?
Once we receive everything, your file then moves on to processing. At that time we review your records to make sure we will be a good fit for you. You can then expect to receiving a phone call from us to book your establishment visit.
